My Online Personal Health Record gives you online access to your Personal Health Record. (Medications, selected labs, and allergies.)
My Online Personal Health Record gathers information from multiple sources including billing information. It is very important that you review this information for accuracy frequently, as the record will be periodically updated from the medical community.

Prescription Renewal Requests are now available over the Internet through Pronger Smith MedicalCare's secure patient portal for all Pronger Smith MedicalCare Providers.
Three easy steps to Access My Online Personal Health Record (PHR)
- You will need to be a Pronger Smith patient to access your PHR generated at Pronger Smith MedicalCare. (Available after 7/18/2011.)
- You must create an account on Pronger Smith MedicalCare’s Portal
- Log into the portal and create your PHR account
The Personal Health Record is a pilot program. We will add 100 patients at a time over the next few months. We want to ensure that we can provide adequate customer service to those who have created a PHR account.

What is My Online Personal Health Record?
Our goal is to provide you with your personal health record which gives you 24/7 online access to your health information. Your Personal Health Record contains your health history including information about allergies, medications and selected lab test results.
How do I access My Online Personal Health Record?
You can view your personal health record through your Pronger Smith MedicalCare’s secure patient portal account. Simply click on the puzzle piece above. You will be asked to create a new account to access your Personal Health Record. (We apologize for the inconvenience of having to create a second account. We are working on a single sign-on solution to be available in the very near future.)

Who can access My Online Personal Health Record?
Selected information will be available to you from the below named institutions and from Blue Cross Blue Shield claims payment system. Currently, your medical information will be available to authorized personnel at Pronger Smith MedicalCare, in the emergency rooms at UIC, Rush and Stroger Hospitals, if you are receiving medical treatment there, when each of those institutions go live. Your insurance company may provide information to My Online Personal Health Record, but cannot access your medical information.

What is available at My Online Personal Health Record?
Medications, allergies and selected lab results from Pronger Smith MedicalCare. Information selected at participating hospital emergency rooms. And Blue Cross Blue Shield insurance claims information. The My Online Personal Health Record content will be subject to change as more information becomes available and more institutions participate.
